Grapefruit not only food that can affect medication

From milk and cookies to chocolate and peanut butter, some foods make a tasty combination. But attention to medical labels, and not your taste buds, is needed when combining certain foods with medications.
Milk and other calcium products, for example, can block the absorption of certain antibiotics. And eating large quantities of chocolate while taking some antidepressants can cause a sharp rise in blood pressure. Even some licorice can reduce the effects of certain blood pressure drugs and diuretics.
A recently published review in the Canadian Medical Association Journal that found an increasing number of prescription drugs could have potentially dangerous interactions with grapefruit and grapefruit juice highlights the importance of consumers being aware of possible food and drug interactions.
It's been known for some time that grapefruit juice can "both increase or decrease the absorption of a small number of drugs," says Hartmut Derendorf, chairman of pharmaceutics at the University of Florida College of Pharmacy. Derendorf was not involved in the Canadian review.
"If the drug is metabolized in the gut wall to a large extent and this metabolism is blocked, then the concentrations in the blood will go up. An example is the lipid-lowering drug simvastatin (Zocor)," says Derendorf. "For other drugs such as the antihistamine fexofenadine (Allegra), grapefruit juice blocks the uptake into the bloodstream and the concentrations in blood will go down."
In all such cases there are alternative medications available that will not interact with grapefruit and the other citrus fruits that contain furanocoumarins, the culprit behind the "grapefruit juice effect," Derendorf says. These include Seville oranges (often used in marmalade), limes and tangelos, a cross between tangerines and grapefruit. Sweet oranges, such as navel and Valencia, do not contain furanocoumarins.
There's more attention than ever before being paid to reducing interactions between food and drugs, Derendorf says, and researchers are focused on finding safe alternatives, removing the chemical compounds that cause interactions, and in some cases, changing the genetic makeup of foods so that they do not interfere with medications.
In December, a team at the University of Florida's Citrus Research and Education Center published a paper about ongoing efforts to create a grapefruit hybrid with significantly reduced interaction risk.
Concerns about the interactions between foods and drugs are not limited to prescription medications. Dietary supplements (which also include vitamins, minerals and herbs) can likewise interfere with how some medications work.
"Often, people think that herbal supplements are benign and don't have interactions, and that's absolutely false," says Christine Gerbstadt, a registered dietitian and spokesperson for the Academy of Nutrition and Dietetics. "Most supplements have the exact same chemicals and ingredients as pharmaceutical drugs but simply in a naturally occurring form or lower dose," she says.
Drugs, of course, can have negative interactions with other drugs, and the same can be true of supplements. Calcium supplements, for example, may decrease the absorption of dietary iron, which is why people at high risk for iron deficiency are encouraged to take calcium supplements at bedtime, instead of with meals.
When being prescribed any medication it's important to inform your physician of all medications you're currently taking — prescription, over-the-counter and dietary supplements — to read the patient information material that comes with the medication and to ask about potential interactions, Gerbstadt says.
"Your pharmacist and registered dietitian can also provide a wealth of information," she says. "No one person can know it all, but huge databases are available to help professionals get access to this information."
A sampling of the potential interactions between foods and medications, including some supplements, according to the Food and Drug Administration:
-- Grapefruit juice: Some statin drugs to lower cholesterol, such as simvastatin (Zocor), atorvastatin (Lipitor), and pravastatin (Pravachol); some blood pressure-lowering drugs, such as Nifediac and Afeditab ; some organ transplant rejection drugs, such as Sandimmune and Neoral (both cyclosporine); some anti-anxiety drugs, such as BuSpar (buspirone); some anti-arrhythmia drugs, such as Cordarone and Nexterone (both amiodarone); some antihistamines, such as Allegra (fexofenadine); the anti-malaria drugs Quinerva or Quinite (quinine); and Halcion (triazolam), a medication used to treat insomnia.
-- Licorice: The sweetening compound glycyrrhizin in black licorice may reduce the effects of some blood pressure drugs or urine-producing drugs including Hydrodiuril (hydrochlorothiazide) and Aldactone (spironolactone). It may increase the toxicity risks from Lanoxin (digoxin), used to treat congestive heart failure and abnormal heart rhythms.
-- Chocolate: Antidepressant Monoamine Oxidase (MAO) inhibitors (such as phenelzine (Nardil, Nardelzine) and tranylcypromine (Parnate) are just one category of drugs that shouldn't be consumed with excessive amounts of chocolate and other caffeinated foods. Caffeine can also interact with stimulant drugs such as Ritalin (methylphenidate), increasing their effect, or by decreasing the effect of sedative-hypnotics such as Ambien (zolpidem). Using bronchodilators with caffeinated foods and drinks can increase the chance of side effects, such as excitability, nervousness, and rapid heart beat.
-- Potassium-rich foods (such as bananas, oranges, and green leafy vegetables): Can add to high potassium levels in the body caused by ACE (Angiotensin Converting Enzyme) inhibitors including captopril (Capoten) and enalapril (Vasotec) prescribed to lower blood pressure or treat heart failure. Too much potassium can cause an irregular heartbeat and heart palpitations.
-- St. John's Wort (Hypericum perforatum): Can reduce concentrations of medications in the blood, including digoxin (Lanoxin), used to treat congestive heart failure and abnormal heart rhythms; the cholesterol-lowering drug lovastatin (Mevacor and Altocor), and the erectile dysfunction drug sildenafil (Viagra).
-- Vitamin E: Taken with a blood-thinning medication such as warfarin (Coumadin) can increase anti-clotting activity and may cause an increased risk of bleeding.
-- Ginseng: May increase the risk of bleeding when taken with anticoagulants (blood thinners such as warfarin and heparin). Can also increase the bleeding effects of aspirin and non-steroidal anti-inflammatory drugs such as ibuprofen, naproxen, and ketoprofen. Combined with MAO inhibitors such as Nardil or Parnate may cause headache, trouble sleeping, nervousness and hyperactivity.
-- Ginkgo biloba: High doses can decrease the effectiveness of anticonvulsant therapy in patients taking seizure-control medicines Tegretol, Equetro or Carbatrol (carbamazepine), and Depakote (valproic acid).

Gatorade Removing Controversial Ingredient But Denies It’s Because Of Teen’s Campaign

A few weeks after a 15-year-old started an online petition to tell Gatorade to get rid of brominated vegetable oil, an ingredient linked to flame retardants, its parent company PepsiCo says it’s pulling the controversial ingredient from the sports drink. But not because of that petition, says the company. Nope, because of everyone else who was complaining.

A spokeswoman made sure the distinction was clear, saying the company began “hearing rumblings” in the past year from consumers concerned about the ingredient, reports the Associated Press. She reportedly said it was not a response to the Mississippi teen’s Change.org petition. Because she’s not a consumer that rumbled in the past year?

The teen’s petition highlighted the fact that brominated vegetable oil has been patented as a flame retardant and is considered unsafe for consumption in Japan as well as by the European Union. As of today the petition she started had more than 200,000 supporters.

PepsiCo’s spokeswoman says its’ used as an emulsifier so that flavoring doesn’t get clumpy. It’s also used in other drinks like Mountain Dew and Coca-Cola products like Fanta and some Powerade flavors.

She says the move is not because the company thinks BVO is unsafe, but to address concerns expressed specifically by Gatorade customers. Again — not, ostensibly, the Mississippi teen’s concerns. Just everyone else’s.

As for the teen, named Sarah, she said via a press release from Change.org that she’s excited about how things have turned out.

“When I went to Change.org to start my petition, I thought it might get a lot of support because no one wants to gulp down flame retardant, especially from a drink they associate with being healthy,” she said. “But with Gatorade being as big as they are, sometimes it was hard to know if we’d ever win. This is so, so awesome.”

“Companies like Gatorade put so much thought into marketing,” she added. “As someone who loves to drink their products, I’m so glad they’re making strides to put as much consideration into their customers’ health.”

She started her petition after researching the ingredients in a Gatorade she was drinking, and was inspired in part by a Scientific American article that linked BVO to ill health effects, especially in children.

Gatorade to remove controversial ingredient

NEW YORK — PepsiCo Inc. is removing a controversial ingredient, brominated vegetable oil, from its Gatorade sports drink in response to customer complaints.

A spokeswoman for the company, Molly Carter, said Friday that the move was in the works for the past year after the company began "hearing rumblings" from consumers about the ingredient. She said it wasn't a response to a recent petition on Change.org by a Mississippi teenager.

The ingredient is also listed in other drinks, including some flavors of Powerade, made by rival Coca-Cola Co. The Atlanta-based company did not say whether it would remove the ingredient from Powerade as well but noted that it takes customers' concerns into account when looking for ways to improve its drinks.

Ingredients in food and drinks have come under greater scrutiny in recent years, helped by the ability of consumers to mobilize online. The petition on Change.org noted that brominated vegetable oil has been patented as a flame retardant and is banned in Japan and the European Union. It had more than 200,000 supporters Friday.

For Gatorade, Carter said the ingredient is used as an "emulsifier," meaning it distributes flavoring evenly so that it doesn't collect at the surface. She said it was used only in select flavors including such as orange and citrus. Brominated vegetable oil, or BVO, is still used in other drinks, including Coca-Cola's Fanta and PepsiCo's Mountain Dew.

A spokesman for competitor Dr Pepper Snapple Group was not available to comment on whether the ingredient is used in any of its drinks.

Carter noted that the ingredient is not banned by the U.S. Food and Drug Administration and that the decision wasn't the result of any health or safety concerns. She said it was to address concerns expressed specifically by Gatorade customers.

PepsiCo is replacing BVO in Gatorade with an ingredient called sucrose acetate isobutyrate, which will maintain the flavor and taste of the drinks.

PepsiCo's decision to remove the ingredient from Gatorade was first reported by the trade journal Beverage Digest.

Baby's birth captured on video made in MRI machine

This is not your average baby video. For one thing, the star isn't even fully born at the end. And, doctors say, this video is the first to use a magnetic resonance imaging machine (MRI) to show birth as it happens, from the inside.

The images were shot during a normal birth at Charité University Hospital in Berlin, Germany, in November, 2010, researchers say in a report published in the American Journal of Obstetrics and Gynecology. Still MRI pictures of the birth were previously published, but this is the first look at the video, made with time-lapsed images over the last 45 minutes of labor.

The MRI was safe for the baby (a boy) and the mother (a 24-year-old woman with two other children), the report says.

There was just one concern: the banging noise familiar to anyone who has had an MRI scan. That's why the video stops just as the baby's head emerges: Doctors turned off the machine because they didn't want to risk exposing his ears to the sound.

The doctors write that their experiment "opened a new way to study the mechanism of birth. " Researcher Christian Bamberg told Reuters in 2010: "The main reasons for the research are to answer the question of why a birth may stall and to visually capture the birthing process and any complications...

The images are spectacular. They show which movements the fetus makes in the birth canal, how its bones move and how its head changes shape during birth."

The doctors don't say how they convinced a woman to go through the final stages of labor in an MRI machine -- but do say she weathered the experience just fine.

Hepatitis B vaccine recommended for adults with diabetes

Hepatitis B vaccination is recommended for all unvaccinated adults with type 1 and type 2 diabetes aged 19 to 59, say new guidelines from the U.S. Advisory Committee on Immunization Practices (ACIP).


The vaccination should be done as soon as possible after adults in this age group are diagnosed with diabetes.

Unvaccinated adults with diabetes who are older than 59 can receive hepatitis B vaccination at the discretion of their doctor, the ACIP advises.

The recommendations are outlined in the Dec. 23 issue of the Morbidity and Mortality Weekly Report, published by the U.S. Centers for Disease Control and Prevention.

Between 700,000 and 1.4 million people in the United States are infected with the hepatitis B virus (HBV), according to background information in the report.

Chronic HBV infection damages the liver and can lead to serious illness and death. More than 15 percent of adults with chronic HBV infection develop cirrhosis and liver cancer, the authors of the report noted.

People with diabetes are at increased risk for HBV infection, which can occur through exposure to small, even invisible, amounts of blood from an infected person who earlier used a shared medical or glucose-monitoring device, the article states.

The hepatitis B virus can survive outside the body and is easily transmitted. This means that virus transmission can occur if finger-stick devices or blood glucose monitors meant for one person are used by more than one person without appropriate cleaning or infection control measures.

"Initiatives are ongoing to improve infection control training of staff responsible for providing or assisting with diabetes care, and to improve the design and labeling of devices used in diabetes monitoring and treatment," according to a CDC news release.
