Prostate cancer screening results differ in men taking cholesterol-lowering statin drugs compared with non-users, a study conducted at Tampere University in Finland finds. In statin users, screening did not increase the incidence of prostate cancer as it did in other men.

The study found the clearest difference in low-risk cancer, which is often overdiagnosed due to screening. The number of low-risk tumours found in screening was significantly lower in statin users. However, statin use caused no difference in the detection of high-risk cancers. In the screened group, prostate cancer mortality was slightly lower than in the unscreened group, both in men taking statins and other men.

"The study provides significant new information because statin use is very common and the effects of prostate cancer screening have not been previously evaluated in relation to statin use," says Professor Teemu Murtola from the Faculty of Medicine and Health Technology at Tampere University.

The study was published in the JAMA Oncology journal on 24 November 2021.

The findings may be explained by the fact that statin use improves the accuracy of prostate cancer screening. This means that screening detects dangerous types of cancer in these men as well as in others, but in statin users, there is less of the so-called overdiagnosis, which means the detection of low-risk prostate cancers that do not pose a health threat due to their very slow growth rate.

Another possible explanation could be that men who use statins are a select group who already actively use health services and have had PSA tests outside systematic screening. In those cases, the additional screening done in the study does not have such a great impact.

The study was based on data from the Finnish Prostate Cancer Screening Trial which started in 1996-1999. A total of about 80,000 men were included in the study, of whom just under 32,000 were screened with the PSA test every four years. The project was carried out jointly by Tampere University and the University of Helsinki and the Helsinki and Tampere University Hospitals.

Eating more nutritious, plant-based foods is heart-healthy at any age, according to two research studies published today in the Journal of the American Heart Association, an open access journal of the American Heart Association.

In two separate studies analyzing different measures of healthy plant food consumption, researchers found that both young adults and postmenopausal women had fewer heart attacks and were less likely to develop cardiovascular disease when they ate more healthy plant foods.

The American Heart Association Diet and Lifestyle Recommendations suggest an overall healthy dietary pattern that emphasizes a variety of fruits and vegetables, whole grains, low-fat dairy products, skinless poultry and fish, nuts and legumes and non-tropical vegetable oils. It also advises limited consumption of saturated fat, trans fat, sodium, red meat, sweets and sugary drinks.

One study, titled "A Plant-Centered Diet and Risk of Incident Cardiovascular Disease during Young to Middle Adulthood," evaluated whether long-term consumption of a plant-centered diet and a shift toward a plant-centered diet starting in young adulthood are associated with a lower risk of cardiovascular disease in midlife.

"Earlier research was focused on single nutrients or single foods, yet there is little data about a plant-centered diet and the long-term risk of cardiovascular disease," said Yuni Choi, Ph.D., lead author of the young adult study and a postdoctoral researcher in the division of epidemiology and community health at the University of Minnesota School of Public Health in Minneapolis.

Choi and colleagues examined diet and the occurrence of heart disease in 4,946 adults enrolled in the Coronary Artery Risk Development in Young Adults (CARDIA) study. Participants were 18- to 30-years-old at the time of enrollment (1985-1986) in this study and were free of cardiovascular disease at that time. Participants included 2,509 Black adults and 2,437 white adults (54.9% women overall) who were also analyzed by education level (equivalent to more than high school vs. high school or less). Participants had eight follow-up exams from 1987-88 to 2015-16 that included lab tests, physical measurements, medical histories and assessment of lifestyle factors. Unlike randomized controlled trials, participants were not instructed to eat certain things and were not told their scores on the diet measures, so the researchers could collect unbiased, long-term habitual diet data.

After detailed diet history interviews, the quality of the participants diets was scored based on the A Priori Diet Quality Score (APDQS) composed of 46 food groups at years 0, 7 and 20 of the study. The food groups were classified into beneficial foods (such as fruits, vegetables, beans, nuts and whole grains); adverse foods (such as fried potatoes, high-fat red meat, salty snacks, pastries and soft drinks); and neutral foods (such as potatoes, refined grains, lean meats and shellfish) based on their known association with cardiovascular disease.

Participants who received higher scores ate a variety of beneficial foods, while people who had lower scores ate more adverse foods. Overall, higher values correspond to a nutritionally rich, plant-centered diet.

"As opposed to existing diet quality scores that are usually based on small numbers of food groups, APDQS is explicit in capturing the overall quality of diet using 46 individual food groups, describing the whole diet that the general population commonly consumes. Our scoring is very comprehensive, and it has many similarities with diets like the Dietary Guidelines for Americans Healthy Eating Index (from the U.S. Department of Agriculture's Food and Nutrition Service), the DASH (Dietary Approaches to Stop Hypertension) diet and the Mediterranean diet," said David E. Jacobs Jr., Ph.D., senior author of the study and Mayo Professor of Public Health in the division of epidemiology and community health at the University of Minnesota School of Public Health in Minneapolis.

Researchers found:

  • During 32 years of follow-up, 289 of the participants developed cardiovascular disease (including heart attack, stroke, heart failure, heart-related chest pain or clogged arteries anywhere in the body).
  • People who scored in the top 20% on the long-term diet quality score (meaning they ate the most nutritionally rich plant foods and fewer adversely rated animal products) were 52% less likely to develop cardiovascular disease, after considering several factors (including age, sex, race, average caloric consumption, education, parental history of heart disease, smoking and average physical activity).
  • In addition, between year 7 and 20 of the study when participants ages ranged from 25 to 50, those who improved their diet quality the most (eating more beneficial plant foods and fewer adversely rated animal products) were 61% less likely to develop subsequent cardiovascular disease, in comparison to the participants whose diet quality declined the most during that time.
  • There were few vegetarians among the participants, so the study was not able to assess the possible benefits of a strict vegetarian diet, which excludes all animal products, including meat, dairy and eggs.

"A nutritionally rich, plant-centered diet is beneficial for cardiovascular health. A plant-centered diet is not necessarily vegetarian," Choi said. "People can choose among plant foods that are as close to natural as possible, not highly processed. We think that individuals can include animal products in moderation from time to time, such as non-fried poultry, non-fried fish, eggs and low-fat dairy."

Because this study is observational, it cannot prove a cause-and-effect relationship between diet and heart disease.

The study was funded by the National Heart, Lung and Blood Institute of the National Institutes of Health; Healthy Food Healthy Lives Institute at the University of Minnesota; and the MnDrive Global Food Ventures Professional Development Program at the University of Minnesota.

In another study, "Relationship Between a Plant-Based Dietary Portfolio and Risk of Cardiovascular Disease: Findings from the Women's Health Initiative (WHI) Prospective Cohort Study," researchers, in collaboration with WHI investigators led by Simin Liu, M.D., Ph.D., at Brown University, evaluated whether or not diets that included a dietary portfolio of plant-based foods with U.S. Food and Drug Administration-approved health claims for lowering "bad" cholesterol levels (known as the "Portfolio Diet") were associated with fewer cardiovascular disease events in a large group of postmenopausal women.

The "Portfolio Diet" includes nuts; plant protein from soy, beans or tofu; viscous soluble fiber from oats, barley, okra, eggplant, oranges, apples and berries; plant sterols from enriched foods and monounsaturated fats found in olive and canola oil and avocadoes; along with limited consumption of saturated fats and dietary cholesterol. Previously, two randomized trials demonstrated that reaching high target levels of foods included in the Portfolio Diet resulted in significant lowering of "bad" cholesterol or low-density lipoprotein cholesterol (LDL-C), more so than a traditional low-saturated-fat National Cholesterol and Education Program diet in one study and on par with taking a cholesterol-lowering statin medication in another.

The study analyzed whether postmenopausal women who followed the Portfolio Diet experienced fewer heart disease events. The study included 123,330 women in the U.S. who participated in the Women's Health Initiative, a long-term national study looking at risk factors, prevention and early detection of serious health conditions in postmenopausal women. When the women in this analysis enrolled in the study between 1993 and 1998, they were between 50-79 years old (average age of 62) and did not have cardiovascular disease. The study group was followed until 2017 (average follow-up time of 15.3 years). Researchers used self-reported food-frequency questionnaires data to score each woman on adherence to the Portfolio Diet.

The researchers found:

  • Compared to women who followed the Portfolio Diet less frequently, those with the closest alignment were 11% less likely to develop any type of cardiovascular disease, 14% less likely to develop coronary heart disease and 17% less likely to develop heart failure.
  • There was no association between following the Portfolio Diet more closely and the occurrence of stroke or atrial fibrillation.

"These results present an important opportunity, as there is still room for people to incorporate more cholesterol-lowering plant foods into their diets. With even greater adherence to the Portfolio dietary pattern, one would expect an association with even less cardiovascular events, perhaps as much as cholesterol-lowering medications. Still, an 11% reduction is clinically meaningful and would meet anyone's minimum threshold for a benefit. The results indicate the Portfolio Diet yields heart-health benefits," said John Sievenpiper, M.D., Ph.D., senior author of the study at St. Michael's Hospital, a site of Unity Health Toronto in Ontario, Canada, and associate professor of nutritional sciences and medicine at the University of Toronto.

The researchers believe the results highlight possible opportunities to lower heart disease by encouraging people to consume more foods in the Portfolio Diet.

"We also found a dose response in our study, meaning that you can start small, adding one component of the Portfolio Diet at a time, and gain more heart-health benefits as you add more components," said Andrea J. Glenn, M.Sc., R.D., lead author of the study and a doctoral student at St. Michael's Hospital in Toronto and in nutritional sciences at the University of Toronto.

Although the study was observational and cannot directly establish a cause-and-effect relation between diet and cardiovascular events, researchers feel it provides a most reliable estimate for the diet-heart relation to-date due to its study design (included well-validated food frequency questionnaires administered at baseline and year three in a large population of highly dedicated participants). Nevertheless, the investigators report that these findings need to be further investigated in additional populations of men or younger women.

Alzheimer’s disease could be delayed or even reversed by giving oxygen therapy to patients in pressurised chambers, a study suggests.


Israeli researchers found that elderly patients with Mild Cognitive Impairment – a forerunner to dementia – had improved memory and brain functioning following regular 90-minute sessions in a hyperbaric chamber.

The pressure change allows more oxygen to be dissolved into the tissues and mimics a state of “hypoxia”, or oxygen shortage, which is known to have regenerating effects.

In separate animal studies, the researchers showed the therapy can help clear away sticky amyloid plaques in the brain, which stop cells from communicating and are a major sign of Alzheimer’s.

The team believes that the treatment works by changing the structure of vessels in the brain so that more blood can get through.

Prof Uri Ashery, of the Sackler School of Medicine at Tel Aviv University, said: “We have discovered for the first time that hyperbaric oxygen therapy induces degradation and clearance of pre-existing amyloid plaques and the appearance of newly formed plaques.

“Elderly patients suffering from significant memory loss at baseline revealed an increase in brain blood flow and improvement in cognitive performance, demonstrating hyperbaric oxygen therapy potency to reverse core elements responsible for the development of Alzheimer’s disease.”

Although experts said the findings needed to be replicated in a larger trial and that such therapy may be difficult to offer at scale, they said it could open doors in using oxygen therapy for the treatment of dementia.

“The paper presents an interesting approach, in particular reminding us of the importance of vascular factors in the development not only of vascular dementia but Alzheimer’s disease too,” said Tom Dening, professor of dementia research at Nottingham University.

More than 64,000 people in Ireland suffer from dementia and that figure is expected to more than double in the next 25 years. The prevalence of the disease is decreasing but the population is ageing.

Hyperbaric oxygen therapy for Alzheimer's disease

Researchers have the first PET scan-documented case of improvement in brain metabolism in Alzheimer's disease in a patient treated with hyperbaric oxygen therapy (HBOT).

Dr. Paul Harch, Clinical Professor and Director of Hyperbaric Medicine at LSU Health New Orleans School of Medicine, and Dr. Edward Fogarty, Chairman of Radiology at the University of North Dakota School of Medicine, report the first PET scan-documented case of improvement in brain metabolism in Alzheimer's disease in a patient treated with hyperbaric oxygen therapy (HBOT). 

The authors report the case of a 58-year-old female who had experienced five years of cognitive decline, which began accelerating rapidly. Single photon emission computed tomography (SPECT) suggested Alzheimer's disease. The diagnosis was confirmed by 18Fluorodeoxyglucose (18FDG) positron emission tomography (PET) brain imaging, which revealed global and typical metabolic deficits in Alzheimer's.

The patient underwent a total of 40 HBOT treatments -- five days a week over 66 days. Each treatment consisted of 1.15 atmosphere absolute/50 minutes total treatment time. After 21 treatments, the patient reported increased energy and level of activity, better mood and ability to perform daily living activities as well as work crossword puzzles. After 40 treatments, she reported increased memory and concentration, sleep, conversation, appetite, ability to use the computer, more good days (5/7) than bad days, resolved anxiety, and decreased disorientation and frustration. Tremor, deep knee bend, tandem gain, and motor speed were also improved. Repeat 18FDG PET imaging one month post-HBOT showed global 6.5-38% improvement in brain metabolism.

"We demonstrated the largest improvement in brain metabolism of any therapy for Alzheimer's disease," notes Dr. Harch. "HBOT in this patient may be the first treatment not only to halt, but temporarily reverse disease progression in Alzheimer's disease."

The report also contains video imaging, including unique rotating PET 3D Surface Reconstructions, which allow the lay person to easily see the improvements in brain function.

"PET imaging is used around the world as a biomarker in oncology and cardiology to assay responses to therapy," says Dr. Fogarty. "We now have an irrefutable biomarker system that this intervention has promise where no other real hope for recovery of dementia has ever existed before."

The physicians report that two months post-HBOT, the patient felt a recurrence in her symptoms. She was retreated over the next 20 months with 56 HBOTs (total 96) at the same dose, supplemental oxygen, and medications with stability of her symptoms and Folstein Mini-Mental Status exam.

According to the National Institutes of Health, "Alzheimer's disease is an irreversible, progressive brain disorder that slowly destroys memory and thinking skills and, eventually, the ability to carry out the simplest tasks. It is the most common cause of dementia in older adults. Alzheimer's disease is currently ranked as the sixth leading cause of death in the United States, but recent estimates indicate that the disorder may rank third, just behind heart disease and cancer, as a cause of death for older people."

The authors note that four pathological processes have been identified and primary treatment is with acetylcholinesterase inhibitors or the N-methyl-D-aspartate receptor antagonist memantine, which have been shown to have a positive impact on Alzheimer's disease progression with no significant disease-modifying effects.

HBOT is an epigenetic modulation of gene expression and suppression to treat wounds and disease pathophysiology, particularly inflammation. HBOT targets all four of the pathological processes of AD by affecting the microcirculation; mitochondrial dysfunction, and biogenesis; reducing amyloid burden and tau phosphorylation; controlling oxidative stress; and reducing inflammation.

The first successful HBOT-treated case of Alzheimer's disease was published in 2001. The present case report is the first patient in a series of 11 HBOT-treated patients with Alzheimer's disease whose symptomatic improvement is documented with 18fluorodeoxyglucose positron emission tomography (18FDG PET). "Our results suggest the possibility of treating Alzheimer's disease long-term with HBOT and pharmacotherapy," concludes Harch.

Face masks critical in preventing spread of COVID-19

A study by a team of researchers led by a Texas A&M University professor has found that not wearing a face mask dramatically increases a person's chances of being infected by the COVID-19 virus.
Renyi Zhang, Texas A&M Distinguished Professor of Atmospheric Sciences and the Harold J. Haynes Chair in the College of Geosciences, and colleagues from the University of Texas, the University of California-San Diego and the California Institute of Technology have had their work published in the current issue of PNAS (Proceedings of the National Academy of Sciences).
The team examined the chances of COVID-19 infection and how the virus is easily passed from person to person. From trends and mitigation procedures in China, Italy and New York City, the researchers found that using a face mask reduced the number of infections by more than 78,000 in Italy from April 6-May 9 and by over 66,000 in New York City from April 17-May 9.
"Our results clearly show that airborne transmission via respiratory aerosols represents the dominant route for the spread of COVID-19," Zhang said. "By analyzing the pandemic trends without face-covering using the statistical method and by projecting the trend, we calculated that over 66,000 infections were prevented by using a face mask in little over a month in New York City. We conclude that wearing a face mask in public corresponds to the most effective means to prevent inter-human transmission.
"This inexpensive practice, in conjunction with social distancing and other procedures, is the most likely opportunity to stop the COVID-19 pandemic. Our work also highlights that sound science is essential in decision-making for the current and future public health pandemics."
One of the paper's co-authors, Mario Molina, is a professor at the University of California-San Diego and a co-recipient of the 1995 Nobel Prize in Chemistry for his role in understanding the threat to the Earth's ozone layer of human-made halocarbon gases.
"Our study establishes very clearly that using a face mask is not only useful to prevent infected coughing droplets from reaching uninfected persons, but is also crucial for these uninfected persons to avoid breathing the minute atmospheric particles (aerosols) that infected people emit when talking and that can remain in the atmosphere tens of minutes and can travel tens of feet," Molina said.
Zhang said that many people in China have worn face masks for years, mainly because of the bad air quality of the country.
"So people there are sort of used to this," he said. "Mandated face-covering helped China in containing the COVID-19 outbreak."
Zhang said the results should send a clear message to people worldwide -- wearing a face mask is essential in fighting the virus.
"Our work suggests that the failure in containing the propagation of COVID-19 pandemic worldwide is largely attributed to the unrecognized importance of airborne virus transmission," he said. "Social-distancing and washing our hands must continue, but that's not sufficient enough protection. Wearing a face mask as well as practicing good hand hygiene and social distancing will greatly reduce the chances of anyone contracting the COVID-19 virus."
